Permission Form to Administer Medicine in School
If your child needs medication whilst at school, we are able to administer this for you. Please complete a permission form and hand both form and medication to the Class teacher. If the medication is prescribed by the Doctor, it must be in the original container with the child's name clearly marked on it and handed to the school office for administering. We are unable to administer any prescribed medication without the proper labels or permission form.
